Premier Health Services LLC is a Minnesota Department of Human Services licensed provider authorized under Chapter 245D to deliver home and community-based services (HCBS). Based at 4740 42nd Ave N, Suite 114, Robbinsdale, MN 55422, our team supports individuals across the Twin Cities metro and surrounding Minnesota communities.

Our service areas span Hennepin County and neighboring counties including Anoka, Ramsey, and the broader seven-county metropolitan region. We work closely with individuals, families, case managers, and support teams to coordinate person-centered services that promote independence, choice, and meaningful participation in everyday life.

As a 245D licensed provider, Premier Health Services offers individualized home supports, homemaker services, adult companion services, respite care, night supervision, 24-hour emergency assistance, individual community living support, individualized home supports with training and family training, and employment development and support services.
